I understand that some treatments at No Limits Medical, LLC may involve hormones, such as testosterone.  Some hormones and therapies, such as GLP-1s,  may affect fertility so I understand that I must notify my provider if preserving or avoiding fertility is important to my healthcare plan. Preserving or avoiding fertility may require additional medications or the elimination of certain medications from your treatment plan. I also understand that No Limits Medical, LLC may have certain policies for treatment plans based on my age, medical history, goals and gender. If you have questions concerning any of these potential issues, you understand that you must notify your medical provider and that your medical provider will explain any potential adverse effects of treatment verbally or in writing. I understand that some treatments at No Limits Medical, LLC may involve medications from a compounding pharmacy that are not FDA approved. In addition, some products may be used off-label from their FDA approval. Your provider can explain any questions regarding specific medications. You are electing to move forward with an appointment with a board-certified provider being made aware of this information.

Purpose: The purpose of this form is to provide you with information about telemedicine and to obtain your informed consent to participate in No Limits Medical, LLC’s telemedicine health service as part of your medical care.

Purpose: The purpose of this form is to provide you with information about telemedicine and to obtain your informed consent to participate in No Limits Medical, LLC’s telemedicine health service as part of your medical care. 


Nature of Telemedicine: Telemedicine involves the use of electronic communications to enable a health care provider and a patient at different locations to share medical information for the purpose of evaluation, diagnosis, consultation, or treatment of the patient. The delivery of healthcare via telemedicine allows the patient and provider to establish a relationship, like they would during a traditional face-to-face appointment. For example, your telemedicine encounter may include interaction through and with the use of the internet, recorded audio communications, physical examinations, medical imaging, medical tests, and diagnoses, as well as related technologies known as “store-and- forward.” 


Benefits: The benefits of telemedicine include improved access to medical services and care, including the expertise of specialists and consultants that may not otherwise be available to you. Telemedicine also permits increased efficiency in evaluations, diagnoses, consultations, and treatment. 


Potential Risks: The potential risks associated with the use of telemedicine are rare but include delays in medical evaluation and treatment due to equipment failures or information transmission deficiencies (such as poor image resolution); breach of privacy of protected health information due to security breaches or failures; and adverse drug interactions, allergic reactions, complications, or other errors due to patient’s failure to provide complete medical information or records. Indemnification YOU AGREE TO INDEMNIFY AND HOLD HARMLESS CIRCLE P HOLDINGS, LLC, NO LIMITS MEDICAL, LLC,  ITS EMPLOYEES, AGENTS, DIRECTORS, MEMBERS, MANAGERS, SHAREHOLDERS, OFFICERS, REPRESENTATIVES, ASSIGNS, PARENTS, PREDECESSORS, AND SUCCESSORS FROM AND AGAINST ANY AND ALL LOSS, DAMAGE, EXPENSE, LIABILITY, CLAIM, OR DEMAND WHATSOEVER, ARISING OUT OF OR RELATED TO ANY FAILURE OF TECHNOLOGY OR EQUIPMENT IN CONNECTION WITH THE PROVISION OF TELEMEDICINE WHETHER OR NOT ANY SUCH LOSS, DAMAGE, EXPENSE, LIABILITY, CLAIM, OR DEMAND ARISES FROM OR RELATES TO NO LIMITS MEDICAL, LLC NEGLIGENCE. 


Alternatives: Alternative methods of care may be available to you, such as in-person services. Your provider may explain any such options to you, if necessary, and you may choose an alternative at any time. 


Follow-up Care - Emergency Situations: In some situations, telemedicine is not an appropriate method of care. If there is an urgent situation, if you have an adverse reaction, if a technical failure prevents you from communicating with your telemedicine provider, or if you believe telemedicine will not provide sufficient safety and quality, you should contact No Limits Medical, LLC. Otherwise, you must seek care at an emergency room facility or other provider equipped to deliver urgent or emergent care. If the situation is an emergency, call 911. Your Privacy Rights No Limits Medical, LLC uses network and software security protocols to protect the confidentiality of your patient health information, including for example your medical record, EMR, imaging, and personal financial data. These protocols are designed to safeguard the data and to ensure its integrity against corruption. Personal information that identifies you or contains protected health information will not be disclosed to any third party without your consent, except as authorized by law for the purposes of consultation, lab ordering, treatment, payment/billing, and certain administrative purposes, or as otherwise set forth in No Limits Medical, LLC’s Privacy Policies. You may withhold or withdraw your consent to the telemedicine health service at any time before or during the consult without affecting the right to future care or treatment, unless you have been determined to be an unfit patient for No Limits Medical, LLC for any reason. You may also withdraw consent to extra personnel participating in telemedicine health services. You may also revoke your consent to allow No Limits Medical, LLC to store and use the video images and audio recordings. The request to revoke consent must be in writing and received by No Limits Medical, LLC. If you revoke your consent, any video images and audio recordings will be destroyed and no longer used by No Limits Medical, LLC. Any uses of videos made with your permission prior to No Limits Medical, LLC’s receipt of your revocation cannot be changed or undone. To revoke your consent to No Limits Medical, LLC’s storage and use of video images and audio recordings of your telemedicine health service, please send a written statement to No Limits Medical, LLC and state that you are revoking your consent for No Limits Medical, LLC to store and use video images and audio recordings of your telemedicine health service. 


Disputes: The parties agree that any dispute arriving from the telemedicine consult will be resolved in Florida, and that Florida law shall apply to all disputes. BY AGREEING TO THIS FORM, YOU AGREE TO AND UNDERSTAND THE FOLLOWING: Telemedicine is the use of electronic information and communication technologies by a health care provider to deliver services to an individual when he/she is located at a different site than the provider. I understand that I need to provide a full and accurate medical history, including any pre-existing conditions, to my telemedicine provider so that my provider can accurately determine what services I need. I further understand that my provider will determine whether telemedicine is appropriate for me at this time, based on the condition being diagnosed and/or treated. I understand that I may benefit from telemedicine, but that results cannot be guaranteed. My provider will inform me who will be present at the provider’s location during the telemedicine service, and I have the right to exclude anyone from being present, if I so choose. I further understand that I have the right to object to the use of a telemedicine service without prejudice to any future care or treatment and without risking the loss or withdrawal of any health benefits to which I am entitled. If there are costs to me associated with my telemedicine encounter, a health care professional will discuss those costs with me prior to the start of my session. Further, I understand and agree that I must pay the full amount of the costs associated with this telemedicine service, including any prescription I may receive, and I will not attempt to submit a claim to Medicare, any other federal payor, or any state or private insurer. I understand that the laws that protect privacy and the confidentiality of medical information also apply to telemedicine, and I agree that No Limits Medical, LLC may provide my confidential personal health information to other medical providers who may be located in other areas, including on rare occasions to providers outside the State, as necessary. I have the right to inspect and obtain copies of all information received and recorded during any telemedicine session, subject to the policies of the physicians, physician assistants, nurse practitioners and facilities involved in my care. I may be charged a fee for copies of my records in accordance with applicable State rules. I have read and understand the information above, and all of my questions have been answered to my satisfaction. I consent to a physician, physician assistant, or nurse practitioner to provide services to me via telemedicine. By clicking or signing “I Agree” on our intake form, I understand and consent to the foregoing acknowledgements and disclosures, including but not limited to No Limits Medical, LLC’s Terms and Conditions and which are incorporated herein by reference. We understand that sometimes appointments need to be cancelled or changed. If you cancel your initial provider appointment more than 24 hours prior to the appointment, we will gladly change your appointment time. Refunds are not available for initial appointments. If you must cancel within 24 hours of the scheduled appointment time, we will gladly change your appointment one time. After 2 missed appointments there will be a $50 rescheduling fee applied that must be paid prior to our clinic rescheduling  you. 


All  purchases of No Limits Medical treatments are final because our products are products that, once sent to the pharmacy for compounding, cannot be used for another patient due to pharmacy regulations. Prescription medications cannot be returned for  reimbursement, and thus we are unable to accept returns for refunds,  even if they are unused. However, we want you to receive  high-quality products that are ready to use. Therefore, we will replace  items if they are defective or damaged. Please message or call us if there is a problem with the order that you received. Please be prepared to take photographs of any damaged item as the pharmacy will often require this in order to send a replacement product. Special rules apply to controlled substances so please message us with any issues and we can determine the best course of action based on the product and the situation. 


All consultations are non-refundable once paid. Labwork is non-refundable once ordered. For auto-renewing products, such as the monthly membership, your payment method will be automatically charged at regular  intervals as described during the checkout process until you cancel your  subscription. You may cancel your auto-payments at any time by calling or messaging us. WE MUST HAVE AT LEAST 48 BUSINESS HOURS to cancel your membership so please plan accordingly. All monthly memberships, whether paid monthly or yearly, are non-refundable once paid. 


Products are shipping using USPS, UPS or FedEx. We will not ship to PO Boxes or commercial addresses. All shipments must be to residential addresses. Products can only be cancelled if they have not yet been sent to the pharmacy. Any products that you have paid for that become unavailable from the pharmacy due to shortage or shipping restrictions will, of course, be refunded.


Please note that, while we try to keep prices as stable as possible, prices are subject to change without notice due to occasional pharmacy or other vendor pricing changes.
