This Privacy Policy explains how No Limits Medical, LLC ("No Limits Medical," "NLM," "we," "us," or "our") collects, uses, shares, and protects information about you when you visit nolimitsmedical.com (the "Site"), complete our online forms, book or pay for services, or communicate with us by email, text, or phone.

Your health information. Health information in your medical record (for example, your health history, lab results, diagnoses, and prescriptions) is protected health information. How we use and share it, and your rights to it, are described in our Notice of Privacy Practices (https://nolimitsmedical.com/nlm-privacy-practices). If this Privacy Policy and the Notice of Privacy Practices differ about your health information, the Notice of Privacy Practices controls.

Who we are. No Limits Medical operates this Site and provides all clinical services. Circle P Holdings, LLC provides certain non-clinical administrative and website services to No Limits Medical as our service provider. Circle P does not provide medical care and may use your information only to perform services for us.

1. Information We Collect

Information you give us. When you complete an intake or refill form, book an appointment, make a payment, or contact us, we may collect:

- Contact details: name, mailing and shipping address, email address, and phone numbers.

- Identity details: date of birth and a government-issued photo ID, used to verify your identity for telehealth care and prescriptions.

- Health information: health history, medications, symptoms, goals, lab results, and other information you share for your care.

- Payment information: billing address and card details. Card numbers are processed by our payment processor; we do not store full card numbers on our Site.

- Communications: messages, emails, and texts you send us, and records of your requests.

Information from others involved in your care. We receive information from laboratories (such as Quest Diagnostics), pharmacies that fill your prescriptions, and other providers you ask us to coordinate with.

Information collected automatically. When you visit the Site, we and our website host may collect basic technical information such as IP address, browser type, device type, pages viewed, referring pages, and the date and time of your visit, using cookies and similar tools. See Section 4.

What we do not collect. We do not collect biometric identifiers (such as fingerprints or face geometry) and we do not ask for your Social Security number.

2. How We Use Information

We use information to:

- Provide, coordinate, and manage your care, including visits, labs, prescriptions, and coaching.

- Schedule appointments and send reminders, lab and refill notices, and account messages.

- Process payments, memberships, renewals, and cancellations.

- Respond to your questions and requests.

- Operate, secure, and improve the Site and our services.

- Meet legal, licensing, and record-keeping requirements.

- Send educational or promotional messages only if you have separately opted in. You can opt out at any time.

3. How We Share Information

We do not sell your personal information or health information, and we do not share it with affiliates or third parties for their own marketing or advertising. We share information only: (a) with your clinicians, pharmacies, and laboratories as needed for your care; (b) with service providers that help us operate (for example, our electronic health record and payment system, secure communications, and IT providers), under contracts that require them to protect it, including HIPAA business associate agreements where required; (c) as required by law or to protect health and safety; (d) in connection with a merger or sale of the practice, subject to this policy; and (e) with your written authorization.

Mobile Information. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties.

We do not share your information with affiliates or third parties for their promotional purposes.

4. Cookies and Analytics

Our Site uses cookies and similar tools to keep the Site working and to understand overall website traffic (for example, which pages are visited most). You can set your browser to refuse or delete cookies; some parts of the Site may not work properly if you do.

We do not use analytics on pages where you enter health information. We do not use advertising pixels on patient intake forms, booking forms, or patient portals, and we do not disclose health information to advertising platforms. We do not track you across other companies' websites for advertising. 5. Your Choices About Communications

- Text messages: Reply STOP to any text to opt out, or HELP for help. You may also opt out by emailing info@nolimitsmedical.com or by text or call to 844-891-8541. Message frequency varies. Message and data rates may apply.

- Marketing emails: Use the "Unsubscribe" link in any marketing email, or email info@nolimitsmedical.com.

- Opting out of marketing does not stop messages needed for your care or account, such as appointment, lab, refill, and billing messages.

6. Your Privacy Rights

Depending on where you live, you may have the right to:

- Access the personal information we hold about you and get a copy.

- Correct inaccurate information.

- Delete personal information, subject to legal exceptions (for example, we must keep medical records for the periods required by law).

- Opt out of the sale of personal information or targeted advertising. We do not sell personal information or use it for targeted advertising.

- Not be treated differently for exercising your rights.

How to make a request. Email info@nolimitsmedical.com or call or text 844-891-8541. We will verify your identity before acting on a request. You may use an authorized agent where the law allows; we may ask for proof of the agent's authority. We will respond within 45 days. If we need more time, as the law allows, we will tell you why.

Appeals. If we decline your request, you may appeal by replying to our decision or emailing info@nolimitsmedical.com with "Privacy Appeal" in the subject line. We will respond to your appeal in writing within the time the law requires. If you are not satisfied with our response to your appeal, you may contact your state attorney general.

7. Children

Our Site and services are for adults 18 and older. We do not knowingly collect information from anyone under 18. If you believe we have, contact us and we will delete it.

8. Data Security and Retention

We use administrative, technical, and physical safeguards designed to protect your information, including encryption of information sent through our Site and forms. No system is perfectly secure. We keep medical records for the periods required by law and keep other information only as long as needed for the purposes in this policy or as required by law. If you have a password for a patient or lab portal, please keep it confidential.

9. Changes to This Policy

We may update this Privacy Policy. We will post the updated policy on this page with a new "Last revised" date. If we make material changes to how we use or share personal information, we will notify you by email or with a prominent notice on our Site before the change takes effect.
