terms and conditions

No Limits Medical, LLC

Last revised October 29, 2026

These Terms and Conditions ("Terms") govern your use of nolimitsmedical.com (the "Site") and the services of No Limits Medical, LLC ("No Limits Medical," "NLM," "we," "us," or "our"). No Limits Medical is a cash-pay telehealth practice. All clinical services, including evaluations, lab review, diagnoses, treatment plans, and prescriptions, are provided by No Limits Medical's licensed clinicians using their independent professional judgment. Circle P Holdings, LLC provides certain non-clinical administrative and website services to No Limits Medical and does not provide medical care.

By creating an account, completing an intake form, booking, or paying for services, you agree to these Terms and to the following, each of which is part of these Terms:

- Privacy Policy: nolimitsmedical.com/privacy-policy

- Notice of Privacy Practices: nolimitsmedical.com/nlm-privacy-practices

- Consents (Telehealth and Compounded Medications): nolimitsmedical.com/telehealth-consents

- Membership Information: nolimitsmedical.com/membership-information

- Cancellation, Refund & Shipping Policy: nolimitsmedical.com/returns%2Frefunds%2Fshipping

If you do not agree, please do not use our services.

IF YOU ARE EXPERIENCING A MEDICAL EMERGENCY, CALL 911 OR GO TO THE NEAREST EMERGENCY ROOM.

1. Who Provides Your Care

Clinical care at No Limits Medical is provided by licensed healthcare providers (“Providers”), which may include physicians or nurse practitioners. Registered nurses (RNs) on our team may provide coaching and care coordination under the direction of your Provider; RNs do not diagnose or prescribe. Your Provider uses independent professional judgment and may decide that telehealth or a particular treatment is not appropriate for you.

2. Where Services Are Available

Telehealth and clinical services are available only in locations where No Limits Medical can lawfully provide them through a licensed Provider. Some services or medications may not be available everywhere. Services are not available outside the United States. If we cannot treat you where you are, we will tell you.

3. Not for Emergencies or Urgent Care

IF YOU ARE EXPERIENCING A MEDICAL EMERGENCY, CALL 911 OR GO TO THE NEAREST EMERGENCY ROOM. If you are having thoughts of harming yourself, call or text 988.

No Limits Medical does not provide emergency or urgent care. Secure messaging is available to members at any time, but messages are reviewed and answered during business hours only (Monday–Friday, 9:00 AM–5:00 PM Eastern Time, excluding holidays) and are not monitored for urgent needs. Do not use messaging, text, email, or voicemail for urgent or emergency concerns. Do not disregard or delay seeking medical care because of anything on the Site.

4. Telehealth Services

Our services are delivered by telehealth, including real-time video visits, secure messaging, and review of lab results and records. Telehealth has benefits and risks, which are described in our Consents (https://nolimitsmedical.com/telehealth-consents). Risks include that information may not be sufficient for a decision (for example, poor video quality), that equipment failures may delay care, that incomplete medical information may lead to drug interactions or other errors, and that, although we use administrative, technical, and physical safeguards, those safeguards could fail. You agree to give us complete and accurate health information, including all medications and supplements you take.

5. Initial Appointment and Labs

Your initial appointment is a one-hour, real-time video visit that is booked and paid in advance. Your initial and annual appointments must be real-time video visits. Required labs are ordered through Quest Diagnostics. The initial appointment fee is separate from membership and is non-refundable once paid. You are under no obligation to become a member after your initial appointment.

6. Prescriptions

A prescription is never guaranteed. Whether any medication is appropriate is decided by your Provider. Prescriptions are for your personal use only. Read all product information and labels, and contact your Provider or the dispensing pharmacy with questions.

We do not call or send prescriptions to local retail pharmacies. Medications we prescribe are filled by United States–licensed compounding pharmacies we work with and shipped to your residential address.

7. Compounded and Off-Label Medications

Some medications we prescribe are compounded by licensed pharmacies or used off-label. Compounded medications are not FDA-approved. Your provider will answer any questions that you have about the benefits and risks for any medication.

Compounded medications are prepared by state-licensed pharmacies for an individual patient based on a prescription, and, where applicable, by FDA-registered outsourcing facilities. Compounded medications are not reviewed by the FDA for safety, effectiveness, or quality before they are dispensed. We prescribe a compounded medication only when your clinician determines it is appropriate for your individual needs. More information is on our Compounded Medications (https://nolimitsmedical.com/compounded-medications) page.

8. Supplements and Fullscript

No Limits Medical may receive compensation for supplements ordered through Fullscript. You are welcome to purchase recommended supplements from any source. Supplements have not been evaluated by the FDA and are not intended to diagnose, treat, cure or prevent any disease. Our Fullscript store (us.fullscript.com/welcome/nolimitsmedical) offers a 10% discount, and you do not need to be a No Limits Medical patient to use it. Discounts may vary by order. Labs and select products not eligible.

9. No Limits Medical Membership, Coaching, and Reserve

No Limits Medical Membership. Ongoing care requires an active No Limits Medical Membership: $99 per month, or $1,089 per year paid in advance. Membership begins when you pay your first medication invoice after your initial appointment. Monthly memberships renew automatically on the same day each month, and annual memberships renew automatically each year, using the payment method on file, until you cancel. For annual memberships, we will send you a reminder by email 15 to 30 days before your renewal date, including the renewal amount and how to cancel. Current membership benefits are described at nolimitsmedical.com/membership-information.

RN-Led Coaching (optional). RN-Led Coaching is an optional add-on for $1,500 per quarter, paid up front. It requires an active No Limits Medical Membership. You are responsible for scheduling your coaching sessions; no credits or discounts are given for unused sessions.

No Limits Reserve. No Limits Reserve is offered on a limited-capacity basis, and enrollment is subject to availability. Reserve pricing and inclusions are described at nolimitsmedical.com/no-limits-reserve and in your Reserve enrollment documents. Reserve does not include emergency, urgent, or after-hours care.

Non-refundable once paid. Membership fees (monthly or annual), RN-Led Coaching fees, and Reserve fees are non-refundable once paid. Initial appointment fees are non-refundable once paid, and labs are non-refundable once ordered. If you cancel, you keep your benefits through the end of the period you have already paid for.

Price changes. We will give you at least 30 days' written notice by email before any change to a recurring membership fee takes effect, so you can cancel before the new price applies.

How to cancel. You may cancel at any time by email to info@nolimitsmedical.com or by text to 844-891-8541. Please give at least 48 business hours' notice before your next billing date so we can stop the next charge; a charge that processes before we receive timely notice is non-refundable. If you cancel, your membership benefits continue through the end of the period you have already paid for, and then end. We will confirm your cancellation in writing.

After cancellation. When your membership ends, we can no longer provide ongoing services such as messaging, labs, appointments, or refills. To support a safe transition, we will make your records available to you and, on request, to your next provider, and we will let you know of any time-sensitive follow-up we are aware of. Cancelling and re-enrolling only to obtain a refill is not permitted, and we may decline re-enrollment if a pattern of cancel-and-restart is used to avoid our policies.

Appointment fees. If you do not show within 5 minutes of your appointment start, or cancel within 24 hours, a $100 fee applies to medical and coaching appointments and must be paid before rescheduling. New patients who no-call/no-show for the first appointment must pay $199 to rebook, and we may decline to accept them as patients. Appointments beyond the 6 included provider follow-ups per year are $200 each.

Medications and products. Medication invoices generally cover a 60-day supply. You will see an itemized invoice before paying. Medications are non-refundable once sent to the pharmacy; damaged or defective items will be replaced, and items a pharmacy cannot fill will be refunded. Prices may change due to pharmacy or vendor pricing; you will always see the price before you pay. We ship only to residential addresses (no P.O. Boxes or commercial addresses). Full details are in our Cancellation, Refund & Shipping Policy (https://nolimitsmedical.com/returns%2Frefunds%2Fshipping).

Payment. You authorize us to charge your payment method on file for fees you agree to, including recurring membership fees until you cancel. You confirm that you are authorized to use that payment method.

Right to decline or end care. We may decline or end a patient relationship for any lawful reason, including safety, nonpayment, or conduct concerns. We will not decline care on the basis of any characteristic protected by law.

10. Cash-Pay Practice; No Insurance

No Limits Medical is a cash-pay practice. We do not bill, accept, or submit claims to insurance, and we do not complete insurance forms or letters of medical necessity. Our services are not insurance, and fees are not insurance premiums. You are responsible for all fees for the services you choose.

11. Your Right to a Good Faith Estimate

You have the right to receive a "Good Faith Estimate" explaining how much your medical care will cost. Under the law, health care providers need to give patients who don't have insurance or who are not using insurance an estimate of the bill for medical items and services.

- You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

- Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

- If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

- Make sure to save a copy or picture of your Good Faith Estimate.

For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises or call 1-800-985-3059. To request a Good Faith Estimate from us, email info@nolimitsmedical.com or text 844-891-8541.

12. Communications, Text Messages, and Opt-Out

By providing your mobile number and checking the applicable box, you agree to receive care-related text messages from No Limits Medical, such as appointment reminders, lab and refill notices, and account messages. Marketing texts are sent only if you separately opt in; consent to marketing is not a condition of any purchase. Message frequency varies. Message and data rates may apply. Reply STOP to opt out or HELP for help, or tell us by any reasonable means, including by email to info@nolimitsmedical.com or by text or call to 844-891-8541. Mobile opt-in information is never shared with or sold to third parties or affiliates for their marketing purposes. You can unsubscribe from marketing emails using the link in any email. We comply with the CAN-SPAM Act and the Telephone Consumer Protection Act.

Electronic communications and signatures. You agree that we may provide notices, disclosures, and agreements to you electronically, and that your electronic acceptance (such as clicking "I Agree") is your signature.

13. Privacy

Information you give us through the Site is governed by our Privacy Policy (https://nolimitsmedical.com/privacy-policy). Your health information is also protected under our Notice of Privacy Practices (https://nolimitsmedical.com/nlm-privacy-practices). We do not sell your personal or health information.

14. Eligibility and Your Account

You must be at least 18 years old to use our services. You agree to provide accurate, current, and complete information, including your name, address, date of birth, phone number, email, and health history, and to keep it up to date. You are responsible for keeping your login information confidential and for activity under your account. Tell us right away if you believe your account has been used without permission.

15. Acceptable Use of the Site

You agree not to: (a) access information not intended for you; (b) probe, scan, or test the security of the Site or any system; (c) use bots, scrapers, or other automated means to access or copy the Site; (d) provide false information or impersonate anyone; or (e) use the Site for any unlawful purpose. Content on the Site is for general information only and is not medical advice for your individual situation.

16. No Guarantee of Results; Testimonials

Individual results vary. We do not guarantee any particular outcome from any treatment, medication, supplement, or program. Patient testimonials reflect individual experiences and are not a promise of similar results.

17. Intellectual Property and Links

The Site and its content are owned by No Limits Medical or its licensors and are protected by copyright and trademark laws. You may not copy, reproduce, or use Site content for commercial purposes without our written permission. This does not limit your rights to your own medical records, which are described in our Notice of Privacy Practices. Links to third-party websites are provided for convenience; we do not control and are not responsible for those websites.

18. Disclaimer for Website Use

The Site is provided "as is" and "as available." To the extent permitted by law, we do not warrant that the Site will be uninterrupted or error-free. This disclaimer applies to use of the Site only and does not limit any duty owed to you in the professional health care services you receive.

19. Limitation of Liability

To the fullest extent permitted by law, No Limits Medical and Circle P Holdings, LLC are not liable for indirect, incidental, or consequential damages arising from your use of this website, and our total liability for website-related claims will not exceed the amount you paid us in the 12 months before the claim. This limitation does not apply to claims arising from the professional health care services you receive, to our gross negligence or willful misconduct, or to any liability that cannot be limited under applicable law.

20. Your Responsibility for Misuse

You agree to be responsible for third-party claims and reasonable costs that result from your own misuse of the Site, your providing false or fraudulent information, or your violation of the law or the rights of others. This section does not require you to be responsible for any loss caused by our own negligence or misconduct.

21. Informal Resolution, Governing Law, and Venue

If you have a concern, please contact us first at info@nolimitsmedical.com; we will try in good faith to resolve it within 30 days. These Terms are governed by the laws of the State of Florida, without regard to conflict-of-law rules, except that nothing in these Terms limits any right or protection you have under the laws of the state where you live or received care that cannot be waived by agreement. Any dispute that is not resolved informally may be brought in small claims court where you live, or in the state or federal courts located in Sarasota County, Florida. Nothing in these Terms limits your right to file a complaint with the Florida Department of Health, the Colorado Division of Professions and Occupations, or any other regulator.

22. General

Events outside our control. We are not responsible for delays or failures caused by events beyond our reasonable control, such as natural disasters, severe weather, power or internet outages, or pharmacy or carrier disruptions. We will work to restore services and keep you informed.

Changes to these Terms. We may update these Terms. We will post the updated Terms on this page with a new "Last revised" date, and we will notify members by email of material changes to fees, renewal, or cancellation terms before they take effect.

Other terms. If any part of these Terms is found unenforceable, the rest remains in effect. Our failure to enforce a provision is not a waiver. You may not transfer your account or these Terms without our written consent. These Terms, together with the documents listed at the top, are the entire agreement between you and No Limits Medical about the Site and our services.
